Thanks to all who replied to this one.  It does seem that the directions 
given in the help files found by George are just a convaluted way of getting 
to run, which I have always done with Windows + r.  Is this a windows 
command or a jaws one?

There is no hotkey to launch Narrator on its own unless you
create one
through the properties of the start menu shortcut. Indeed,
Windows-U
will launch the utility manager and also launch Narrator (by
default),
however you do not change those settings in accessibility
options;
they're changed through utility manager itself.
