Billy, that's a rather loaded question, as what's best is an individual
judgement. The built-in eSpeak synth and its attendant voices works fine for
some, while I find it a nightmare.
NVDA will work with most SAPI4 and SAPI5 compliant voices, which gives you a
wide choice.
There are some suggestions on the NVDA site about other speech synthesisers
that will work with NVDA, with installation instructions:

Factors that might influence your choice include what you use a screen
reader for, and how fast you want to run it, how powerful your PC is, not to
mention how much you're prepared to pay. Searching for something like "text
to speech voices" will unearth
plenty of options.
This is a terrible generalisation, but the prettier voices may be less
responsive than older robotic ones.
All I can suggest is that you download all the free voices you can get hold
of, and try the demos on sites such as Nuance, cepstral and CereProc, to
name but a few.
