Both current iPod touch and iPhone have VoiceOver built in.  It can be 
activated and configured when you first connect your device to iTunes software. 
 This process is an essential part of setting up the device so you will need to 
install iTunes as a matter of priority unless you get it set up in the Apple 
store assuming that is where you are buying from, if you purchase from a phone 
carrier or elsewhere you will have to carry out this process yourself.  The 
iPHone works on exactly the same principle as the iPod Touch plus it has a 3G 
and GPS receiver and better camera, therefore provides extra facilities.

Which you go for depends on your level of commitment to mastering the device 
and also whether you are happy to take on the financial commitments involved in 
an iPhone on contract usually.
