Coming up with key phrases is imperative and it's not too late to change
these now.  It's harder to get directories changed but search engines will
spider the site and adjust their directories accordingly (that's why sites
move up and down in rankings regularly).

To properly optimize you will need to identify relevant key phrases and then
optimize both the content and the page titles to start.  I would also be
skeptical of optimizing for a phrase like "geriatric care". It's a very
general term and will likely be extremely competitive and difficult to
unseat the top ranking sites.  Also, is the site a general site about
geriatric care? Things to consider in creating the key phrase(s) are what
service do you offer, where do you offer that service, what do you want to
get out of optimization, and for whom are you optimizing (i.e. what audience
do you want to attract).

as well as modifying the <title> tags to include any relevant
keywords{such as: geriatric care}

The file name and title tag information are two of the most important
considerations for SEO optimization.

I suggest applying the same rationale to the rest of the site, keeping
in mind what keywords a prospective client would use when googling.
