In theory, our company has an internal "ticketing" system that all the
people on the project can access, assign tasks, and report the progress of a
task.  Again in theory, this is an excellent tracking tool for bugs that
keep appearing.  However, I found myself out of the loop on most "issues"
that happened with the project because people automatically assigned the
tasks without my knowledge.  (And then my clients also kept finding "issues"
that were really "enhancements"...)

So in Practice, I find that my Excel document works much better.  I give a
blank template to each of the people testing the project, including the
client, which I then combine, assign tasks, and check progress.  (It's so
rewarding to be able to physically cross/check items off the list.)  The
template I send out includes Section Title, Page Title, Task area (design,
navigation, programming, content...), description, and comments.  My task
list includes priority, date needed by (somewhat redundant, but management
required it), and that glorious check box for completed.
