I'm going to add just one more that doesn't quite fit into the original
specs of the question, but I like what they offer.  This is a group called
JUMPLINE.com.  For what they offer, it's a good "middle of the road" price.

(They also offer Frontpage Extensions, but I think that fits in the
"roadkill in the middle of the road.)
