Ray,

I would say no there is no general industry standard.  Though I do agree
with your comment on which is most common.

My basis for this: Of the many companies that now produce multiport
network analyzers, there was one major provider that changed between the
two numbering schemes based on the calibration algorithm chosen.

Now, nearly all VNA providers allow you to renumber the measurement
ports to your preferred convention.  So some degree of a general
industry convention may be feasible.

It has been my observation that scheme a) is the most common and
intuitive (when I think S12 I think transmission and when I think S13 I
think crosstalk) however scheme b) has popped up often enough to be a
possible contender.
