deck talk could be used for it if a driver is written for NVDA to use it,
for mouse control you can turn on mouse keys. Mouse keys is a feature
built into the accessibility options of Windows XP.

Can the DECTalk Express on Com1 or more be added to NVDA? How do I learn
mouse movement keys? I find JAWS mouse keys easier to use since they use
the numeric keypad on a desktop keyboard and work well with my
nerological uniqueness.
