Printing is done through the print.css style sheet. Feel free to make 
improvements.

This is usually not very complicated I did this for a WakkaWiki install some 
time ago (see http://wiki.splitbrain.org/wakka2doku.pl)

No such thing doesn't exist yet. Contributions would be welcome.
