It is not the easiest of forms to complete, but I went through the process
of almost creating another account just now and it was doable.

Try going to this link to get started which I hope will work for you if you

I've found music store, but cannot seem to find how to set up an account
from there.

I was able to set up my iTunes account without sighted help using Internet
Explorer.  If you just log onto the Apple store from your country, you can
do it there.
I think for the UK it is

Since you have sighted help available, try setting up the account without
JAWS.  I use Window-Eyes and had sighted help.  With Window-Eyes turned off,
it was possible to set up the account.    I could not have done it without
sighted help.
