I checked.  Arizona does indeed regulate class IIIb lasers (>5mW), look 
under the Arizona Radiation Regulatory Agency (ARRA at 
http://www.arra.state.az.us/).  Yes, the same guys who certify nuclear 
labs and power plants for the state.  Typical bureaucratic overkill is 
involved.  The licensing requirements are prohibitive to an individual 
for our types of uses... yearly fees ($350), forms, site license, 
training requirements, etc.
