While reading the incoming message, press [Reply]. You are now in
the internal editor. Press [External Editor]. You now have the
reply in the external editor.

When you have finished composing your reply, exit the external
editor. You are now back in the internal editor. Press [Send mail
NOW!] or [Save to Outbox] to send or save the message.

So how do you change the message from a CNM to a TBS? Surely you
wouldn't go through the screen that opens up the message with the
internal editor? That would invalidate all of your arguments!

Yes, I should turn on that feature. But I have it turned off for
various reasons when I use QEDIT for other purposes. But I
suppose I could create another copy of QEDIT with it turned on...
