The only PCI modem one I have found which works flawlessly
is the one I am using now.
ActionTech PCI Master #pci56012-02
It has no on-board jumpers and does not get a comm port # assigned.
However, by running pmdmcfg.exe at the DOS prompt, the modem is detected
and we are given the base port address and IRQ
Found PCI V.90 Data/Fax Modem:

(pmdmcfg.exe was included with the modem when I purchased it on ebay)

Everything works like a charm with those settings.
